 It  was brought to the attention of the TDOA that changes to the Optometry Law were requiring that to be valid that the RX must include a inter pupillary distance, multifocal type, and tint color to be valid. You will have to download the file, fill it out and then mail it back to the board at 665 Mainstream Drive 2nd Floor Nashville, TN 37243. Don’t forget to get it done before the end of the year!
